Radiant Images - Rental Forms

Thanks for your interest in renting from Radiant Images.

To better assist you, we've included the following forms to read and fill out prior to coming to the facility. This expedites the rental process, helping you get your equipment faster and more efficiently.

A good starting point is our Rental Requirements page, which outlines our insurance requirements, deposits and payment terms.

Also below is a form to fill out if you are setting up a New Account, as well as a Credit Card Authorization form and, of course, the Rental Agreement.

Once completed, please fax them to us at 310.861.0163 along with a copy of the credit card and the cardholder's driver's license that was included information for in the forms.
